Referrals
We welcome referrals from both existing and new circle-of-care providers. You may be a legal, healthcare, social service, religious/spiritual, or a community organization, which is seeking to provide care to individuals in crisis, dispute or conflict; Alternative Decisions is here to provide you and your clients a solution.
How To Submit a Referral.
There are two documents that need to be completed and submitted, in order to refer an individual for services.
Disclosure Consent Agreement - To be completed by the individual being referred.
Referral Form - To be completed by the individual making the referral, and only after the Disclosure Consent Agreement is completed. It is at the discretion of the individual making the referral to include the individual being referred in the completion of the Referral Form.
When submitting a referral, both documents must be completed and submitted together. Submissions lacking both documents will not be reviewed.
Referrals may be submitted in one of the following ways;
